Community-Based Services

Some challenges require more support than a weekly office visit can provide

HWC community-based behavioral health services bring structured support into the environments where everyday challenges occur. Depending on the program and clinical need, services may take place in the home, school, community, or other appropriate settings.

Community Support Team (CST)

Community Support Team provides intensive, team-based, community-based support for eligible adults whose mental health or substance use needs significantly affect daily functioning, stability, and independence. Rather than relying only on office-based appointments, the CST team works with individuals in the community to address behavioral health needs and practical barriers.

Not Sure Which Level of Care Is Appropriate?

Families and referral partners do not need to determine the right service on their own. HWC can discuss what is happening, review available information, and help identify an appropriate next step based on clinical need and eligibility requirements.
